What the research found
Longitudinal cohort work following women through the transition documented small but genuine declines in verbal learning and processing speed during perimenopause, with performance largely rebounding afterwards. Women were not imagining it, and they were not developing dementia.
Brain imaging shows a shift in cerebral glucose metabolism across the transition, alongside changes in the estrogen receptor network in regions supporting memory. The brain appears to be adapting its fuel strategy.
How much is sleep
Night sweats fragment sleep, and fragmented sleep degrades exactly the functions women report losing: word finding, working memory, attention. In many studies, controlling for sleep and mood shrinks the cognitive effect substantially.
That is good news, because sleep is more tractable than hormones for most people, and improving it produces noticeable cognitive gains within weeks.
What to do about it
Stabilise blood sugar. The brain runs on glucose; large swings feel like fog. Protein-forward meals and post-meal walks help within days.
Prioritise DHA. Oily fish twice weekly or an algae-based supplement for plant-based eaters. DHA is a structural component of neuronal membranes.
Train cardiovascular fitness. Aerobic exercise raises BDNF and is one of the few interventions with consistent cognitive benefit across trials.
Protect sleep aggressively: cool room, consistent wake time, alcohol moderation, and a real conversation with a menopause-literate clinician about vasomotor symptoms and hormone therapy if they are driving the disruption.
Learn something hard. Novel, effortful learning builds cognitive reserve in a way that puzzle apps do not.
